WEDI CAQH Audiocast - Proposed CORE Phase II Connectivity Rule: Narrowing the Communication Gap in Healthcare Data Exchange

In this session, representatives from Siemens and Harvard Pilgrim HealthCare will provide a detailed review of the proposed CORE Phase II Connectivity Rule and its potential implications for the future of electronic healthcare information exchange. They will also share their perspectives and experience with participating in the CORE Rule development process and the drive toward implementation of the Rules.

The CORE Phase II Connectivity charge was to create a definitive rule that would further facilitate interoperability. Such interoperability is expected to improve efficiencies and utilization of electronic transactions, and ultimately lower the administrative costs for healthcare providers and payers. This proposed Phase II Connectivity Rule expands on the foundation of Phase I, and like the Phase I Connectivity Rule, is intended to be a Safe Harbor (i.e., all CORE-certified entities must support requests to implement this specified connectivity mechanism, but they can also use other methods).

Although it is expected that the primary use of proposed CORE Phase II Connectivity Rule will be for electronic administrative transactions, it is designed to be “payload agnostic”. Therefore, the same envelopes can be used to carry clinical data as well. Being payload agnostic was a key criterion for CORE as it supports interoperability. Moreover, by supporting SOAP+WSDL enveloping, this proposed CORE Rule is well aligned with the direction of HITSP and HL7, and lends itself to future rule development using more advanced Web-Services standards.

Proposed CORE Phase II Connectivity Rule


As with all CORE rules, rather than create new standards, the proposed CORE Phase II Connectivity Rule uses existing open standards. The Rule provides domain specific connectivity/security information for electronic administrative transactions. It not only specifies the envelope standards, but also the envelope metadata, their syntax and semantics. Two open standards for enveloping – SOAP+WSDL, and HTTP MIME Multipart are used in the Rule. Two authentication standards – X.509 Certificates over SSL, and Username+Password Web-Services Security token are supported. Further, the rule provides error handling, auditing, and detailed envelope examples.

The envelope and authentication standards supported in the proposed Phase II Connectivity Rule present certain implementation options. To facilitate interoperability, conformance requirements for both Real-time and Batch transactions for Healthcare Providers, Payers, Clearinghouses and Software Vendors are clearly defined in the proposed Rule.

Expanding on foundation of Phase I, the proposed CORE Phase II Connectivity/Security Rule addresses the following:

  • Message Enveloping Standards & Specifications
    – SOAP+WSDL
    – HTTP MIME Multipart
  • Submitter Authentication Standards
    – Username/Password
    – X.509 Certificates over SSL
  • Error & Audit Handling
  • Envelope Metadata Fields & Definitions
